About the Role
The Lymphoedema Practitioner role encompasses an outpatient Lymphoedema Clinic and delivery of clinical services at Kyabram Health. The Lymphoedema Practitioner will be responsible for the provision of person- centred care coordination, including clinical assessment and treatment, self-management education, support and referral to consumers living with Lymphoedema, their carers and families. You will be responsible for managing and maintaining a client database, inclusive of waitlists and ongoing follow up needs of clients. Manage booking system with triaging of need based on referral, maintain a diary system of appointments within PAS with accurate referral codes to ensure accurate reporting data for service.
About You
To be successful in this role you will need a minimum of 2 years recent post graduate experience in clinical nursing or allied health specialty; demonstrated diverse and advanced communication skills, ability to be flexible in meeting individual patient, family and carer needs. You need to be able to work autonomously, whilst remaining part of the Cancer Services team and multidisciplinary team. Registration with APRHA in your current field and the National Lymphoedema Practitioners Register is a mandatory requirement.
