Extended Care Model:
KnG Healthcare’s Extended Care is specifically tailored to accommodate older patients who require healthcare services for an extended period of time. These patients continue to require health care in a clinically supported environment to assist with their complex discharge needs following an acute inpatient stay. Our model creates a restorative and healing environment for patients, prioritising their well-being and continuing their healthcare while simultaneously progressing discharge plans.
About the role
Permanent full time and part time opportunity to deliver quality; evidence based Social Work clinical services in the assessment, treatment and discharge planning of older consumers with complex health issues, their families, carers and relevant stakeholders, in a sub-acute setting.
- As a member of the interdisciplinary team, deliver competent level Social Work clinical services to older consumers with complex health needs, in accordance with professional practice, ethical and the National Safety and Quality Health Service Standards (NSQHS) to ensure culturally appropriate consumer focused outcomes.
- Actively participate as an important member of the interdisciplinary team by providing clinical and discipline advice and contributing to the development of treatment, service provision and discharge planning.
- Participate in the identification, development and implementation of quality and service improvement activities that continually improve consumer care and enhance the delivery of services.
- Contribute to the health equity agenda and meet the intent of supporting the defined six actions that specifically meet the needs of Aboriginal and Torres Strait Islander people within the National Safety and Quality Health Service Standards (NSQHS), by actively supporting the elimination of racial discrimination and institutional racism; supporting increased access to health care; influencing the social, cultural and economic determinants of health; supporting the delivery of sustainable, culturally safe and responsive health services; and recognise the importance of working with Aboriginal and Torres Strait Islander peoples, communities and organisations to design, deliver, monitor and review the health and support services we provide.
Requirements:
- Registration with the Australian Association of Social Workers (AASW).
- Compiance with the AASW education requirements - a bachelor's degree in the discipline or an approved undergraduate degree and a master's degree in social work.
- Proficiency in clinical management and professional practice.
- Well-developed communication and interpersonal skills.
- Sound computer software and database entry skills.
Essential Documentation/Declarations:
- Police Check with no disclosable offences, ensuring suitability for employment.
- Australian Workers Rights documentation, affirming eligibility for employment in Australia.
- Compliance with Vaccine Preventable Diseases (VPD) Certification, prioritising patient and staff safety.
