About the role:
Juniper Sarah Hardy, located in Kelmscott, is seeking a Registered Nurse to join our supportive residential aged care team.
In this role, you will assess residents’ clinical needs, coordinate care with healthcare professionals, and implement and review person-centred care plans. You will also provide guidance to care staff and help ensure high standards of clinical care for residents.
We are seeking a highly motivated Registered Nurse to join our team as a Nurse In-Charge. The ideal candidate will have proven experience in supervisory or leadership roles within a clinical setting and possess strong organisational and team-management skills.
SHIFT INFORMATION:
Wednesday: 22:15pm-7:15am
Thursday: 22:15pm-7:15am
Friday night shift 22:15pm -7:15am
Key Responsibilities:
- Conduct clinical assessments and respond to changes in resident health and wellbeing
- Develop, implement and review person-centred care plans in partnership with residents and their families
- Administer medications and provide clinical treatments in line with professional standards
- Provide leadership, mentoring and guidance to care staff
- Liaise with GPs, allied health professionals and external providers to coordinate resident care
- Maintain accurate and timely clinical documentation and reporting
- Support AN-ACC assessments and ensure accurate clinical documentation
- Ensure compliance with Aged Care Quality Standards, policies and best practice
- Participate in infection prevention, risk management and quality improvement initiatives
Key Requirements:
- Current AHPRA registration as a Registered Nurse
- Supervisory/RN in charge experience (essential)
- Strong clinical assessment and care coordination skills
- Ability to guide staff in wound and diabetes management
- Sound understanding of medication systems
- Excellent communication, teamwork and problem-solving skills
