About the role:

We are currently seeking a Physiotherapist to join our supportive team at our Juniper Cygnet, aged care home, located in Bentley.

You will play a vital role in our multidisciplinary team, offering high quality, resident focused physiotherapy services.

This role requires working two days per week Wednesday and Friday from 8:00am- 4:00pm.

Key Responsibilities:

• Conduct comprehensive assessments, develop personalised care plans, and implement effective strategies to meet each client's unique needs
• Design both individual and group exercise programs to keep clients motivated and engaged to optimise their function and independence
• Through equipment assessment and prescription, identify and provide solutions that support client autonomy and well-being
• Foster collaboration and successful outcomes by communicating effectively with clients, families, and caregivers
• Ensure seamless care coordination and accurate progress tracking through meticulous record-keeping
• Work closely and collaboratively with the multidisciplinary team and Therapy Assistants to achieve the desired outcomes for your clients

Key requirements:

• Current full registration with AHPRA as a Physiotherapist (essential)
• Previous experience working within aged care 
• Strong interpersonal and communication skills
• Proven ability to assess, plan, implement, and evaluate quality care
