Exciting new opportunity for an experienced Monday to Friday afternoon (14:00-22:00) Clinical Care Coordinator & Nurse Mentor to join the Chamberlain Gardens Clinical Team.

This newly created leadership position has been introduced to further strengthen clinical support, enhance team capability, and drive excellence in resident care. As an additional Clinical Care Coordinator & Nurse Mentor, you will play a key role in providing clinical leadership during the afternoon shift, mentoring and developing nursing staff, and ensuring the delivery of high-quality, person-centred care.

Working closely with the broader clinical leadership team, you will have the opportunity to influence best practice, support professional growth across the team, and contribute to a positive and collaborative workplace culture. This is an exciting opportunity for a passionate Registered Nurse looking to combine clinical expertise with leadership and mentorship in a rewarding aged care environment.

Key Responsibilities

  • Provide strategic and operational leadership to ensure high‑quality, resident‑centred clinical care.
  • Oversee development, implementation, and evaluation of individualised care plans aligned with best practice.
  • Mentor and support RNs, ENs, and PCWs to strengthen clinical capability and teamwork.
  • Coordinate multidisciplinary collaboration with allied health, GPs, hospitals, and families for holistic care.
  • Monitor clinical activities and daily routines (including mealtimes) to maintain service excellence.
  • Lead effective communication processes such as handovers, clinical huddles, and team meetings.
  • Manage complex resident care needs, including specialist referrals, case reviews, and escalation of health changes.
  • Drive staff development through onboarding, training identification, coaching, and performance support.
  • Oversee quality and risk activities, including audits, incident management, documentation, and improvement initiatives.
  • Implement strategies to minimise clinical risks (falls, infections, adverse events) and promote resident well‑being.
  • Coordinate daily workflow, resource allocation, and communication with senior leaders to ensure efficient operations.

You will offer?

  • You will have an innate empathy for our residents and be committed to continually improving the quality of clinical care provided to them.
  • Minimum 1 year experience as a Care Coordinator or similar leadership position
  • Must be available Monday to Friday 2pm to 10pm
  • Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)
  • Post graduate qualifications in aged care would be advantageous but not essential.
  • Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC and accreditation compliance and documentation.
  • Engaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
  • High level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
  • A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
  • Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
  • Have a valid NDIS Workers Screening Clearance (or ability to apply via How to apply | NDIS).
