As Care Manager you will be responsible for ensuring a high standard of clinical care and nursing services are provided to residents within our residential aged care homes. You will be working as the 2IC or “2nd In Charge” to the Director of Care and Services, providing a high level of clinical support to the clinical services team.

You will be responsible for:

  • Supporting the Director of Care and Services to manage the delivery of high quality clinical services, providing clinical leadership to the registered nursing and care service teams
  • Providing oversight of Standard 2, ensuring appropriate actions are taken, reviewing documentation and closing out any incident notes
  • Facilitating the smooth entry of new residents into the aged care facility, supporting them orientate to their new surroundings within the first few days
  • Ensuring resident care plans are reviewed at least 3 monthly by maintaining the schedule of review for all residents, assigning care plan reviews due each month to Registered Nurses and assisting/monitoring completion of same
  • Collaborating with residents and their families to ensure high levels of satisfaction, proactively resolving issues and facilitating family case conferences at least annually.
  • Acting as Infection Control Officer, managing employee education, maintaining outbreak kits, etc
  • Supporting the Director of Care and Services to ensure the clinical services team has the appropriate level of qualifications, capabilities and experience to deliver the level of care required
  • Coordinating training for team members, including orientation for new employees, on-the-job coaching, and safety training
  • Coordinating the process of assessment from admission to ACFI submission for each resident
  • Ensuring all mandatory ACFI submissions are completed by the due dates as well as reviews for possible uplift undertaken at each resident’s 12-month reappraisal date or whenever there is a major change in care needs
  • Managing clinical stock replenishment, ensuring there are sufficient supplies of clinical dressings, nutritional supplements, incontinence products, etc
  • Maintaining your duty of confidentiality relating to residents, volunteers and employees, ensuring that any confidential information that becomes known through the course your employment remains confidential
  • Creating a positive work environment, demonstrating the Greengate values in your daily actions

Successful candidates possess the following:

  • Completion of tertiary qualifications in nursing, healthcare, or similar (Bachelor of Nursing)
  • Current Registered Nurse registration with AHPRA
  • Thorough understanding of the Aged Care Legislation and Aged Care Quality Standards
  • Demonstrated understanding of ACFI assessment processes
  • Strong clinical knowledge and understanding of the ageing process, particularly age-related health issues
  • Previous experience working within a residential aged care facility in a similar role as Clinical Nurse or ACFI Coordinator
  • Strong coaching / training skills with the ability to positively influence other healthcare workers
  • Effective conflict resolution skills with a high emotional intelligence
  • Strong customer-centric approach to business
  • Ability to work independently and as part of a multi-disciplinary team
  • Commitment to respecting the values, customs, preferences and spiritual beliefs of residents and their families/representatives
  • Ability to read, write and communicate effectively in English
  • Competent using computer systems, including Microsoft Outlook and eCase
  • Genuine interest in working closely with elderly residents within an aged care environment
  • Current Police Check Certificate (valid within last 12 months)
  • Current Influenza Vaccination Certificate (valid within last 12 months)
