We have an exciting opportunity to join the team as a clinical care partner to lead clinical case management and oversee high quality care delivery for our consumers in their own home. You will aim to achieve the best care outcomes for each client from their funded aged care services.
The Role:
Reporting directly to the General Manager Home Care you will be responsible for managing the entire lifecycle of Home Care Packages (HCP) and Support at Home (SaH) while providing excellent customer service for consumers and adhering to the Aged Care Standards.
You will onboard and provide oversight of a caseload of Doutta Galla consumers through your comprehensive assessment skills. Furthermore, you will utilise your extensive knowledge (training provided) of Commonwealth Home Care Packages to optimise a consumer's home care journey.
Key Responsibilities:
The Care Partner – Clinical, develops and implements care plans in accordance with their care and support needs including:
- Onboarding of consumers
- Care planning and coordination
- Review, assess and evaluate of the provision of care
- Develop and maintain care plans and budgets aligned to their aged care funding
- Liaise with relevant service providers
Experience and skills:
- Experience in working in a community aged care setting.
- Clinical knowledge that encompasses clinical assessment, prevention, planning, treatment, management, and review to optimise quality of life, reablement and maintenance of older persons living in the community
- Advanced clinical assessment and decision making are essential for evaluating consumer needs and managing risks
- Provide expert clinical consultation on complex cases.
- Strong planning, time management and organisational skills with the ability to prioritise accordingly
- Sensitivity in addressing consumer’s emotional and physical needs.
- High level interpersonal and written skills including the ability to liaise with consumers, consumers representatives, and other health professionals.
- Efficient time management for handling multiple clients and tasks.
- Proficiency in Microsoft Office (Word and Excel); knowledge of Alayacare is highly desirable
