About Us

Homewell is part of the broader Kinyara Health family, a growing group of community-based care providers dedicated to keeping personal, local care alive for ageing Australians. Kinyara's mission is to help people find their forever home care provider through trusted local brands that combine deep community connections with innovative systems and support.

At Homewell, we believe great aged care is built on relationships. That's why we've created a care model that empowers our Care Partners to develop genuine, long-term connections with the people they support, rather than being caught in a cycle of constant caseload changes and transactional care management. This commitment allows our clients to experience continuity, trust and personalised support throughout their care journey.

As one of Kinyara's fastest-growing brands, Homewell continues to expand its highly experienced Care Partner team to meet growing demand accross the community. Supported by strong leadership, clinical expertise, modern systems and a collaborative team culture, Homewell has developed a proven approach to helping older Australians remain independent, connected and thriving in their own homes.

Our team includes experienced Care Partners, clinical support from Registered Nurses, and professionals with deep expertise in Home Care Packages and the evolving Support at Home landscape. Together, we're focused on delivering meaningful outcomes for clients while creating an environment where employees are supported, trusted and encouraged to grow their careers.

What makes Homewell different is our belief that great care starts with great people. We provide the flexibility, tools and support our team needs to do their best work, while remaining true to the values that underpin Kinyara Health: kindness, excellence and a commitment to creating better outcomes for every client, every day.

Why it's different

Let's be honest… Many care management roles become a constant cycle of administration, caseload reshuffling and reactive problem solving.

At Homewell, we've worked hard to build something different.

You'll join a highly experienced team of Care Partners who are passionate about creating meaningful outcomes for clients. Rather than constantly moving clients around between team members, we empower our Care Partners to build long-term relationships and become a trusted partner throughout their care journey.

Our team is supported by strong systems, clinical expertise, experienced leadership and a clear model that works.

You'll have:

·        A highly experienced team around you, including clinical support from an RN.

·        Flexibility to manage your workload with one weekly anchor day on Monday.

·        Genuine work-life balance with business hours and autonomy.

·        Career development opportunities as Kinyara Health continues to grow.

·        A culture centred around ensuring every client feels supported from day one.

About The Role

We’re looking for a passionate, adaptable and commercially minded Care Partner to join our team and support aged care participants under the Support At Home program.

As a Care Partner with Homewell, you’ll move beyond coordination - building trusted, face‑to‑face relationships with participants and their families, acting as their guide, advocate and strategic partner throughout their care journey.

You’ll focus on outcomes: helping people set meaningful goals, anticipate changing needs, proactively adjust care plans, and make informed choices about how to use their funding in ways that genuinely enhance their daily lives.

You’ll be supported by experienced colleagues and a dedicated manager, clear processes and smart digital tools (including AI‑enabled supports) that free you up to focus on what you do best - listening and connecting, solving problems and delivering high‑quality, goal‑driven care across a defined caseload of 75 participants.

This is a role for someone who enjoys learning, is comfortable with change, and wants to help shape how great care is delivered in an evolving sector.

Key responsibilities include:

  • Translate participant goals into clear, practical care plans that are personalised, compliant, and easy to understand.
  • Keep care plans current through regular reviews, proactive check‑ins and timely updates as participant needs change.
  • Actively plan and track participant budgets to support goal achievement, sustainability and value for money.
  • Guide participants through informed decision‑making around services, priorities, trade‑offs and co‑contributions.
  • Identify emerging risks, changes in wellbeing or service gaps early and take timely action.
  • Coordinate services and referrals to ensure care is seamless, reliable and responsive
  • Maintain accurate, high‑quality documentation and records that are always current and audit‑ready.
  • Manage incidents, feedback and complaints with professionalism, empathy and timely follow‑up.
  • Work closely with care workers, clinicians and internal teams to deliver consistent, high‑quality care.
  • Use systems and digital tools effectively to stay organised, meet timeframes and maximise client‑facing time.

About You

You care deeply about people - and you also enjoy thinking, planning and problem‑solving. You bring empathy, confidence and resilience, and you take pride in helping older people live safely, independently and with dignity at home.

You enjoy partnering with participants rather than “doing things for” them. You’re comfortable having meaningful conversations, including budgets and services, and you approach change as an opportunity to learn and do things better.

Most importantly, you bring a caring heart and a commercial mind - balancing kindness with accountability, and warmth with sound judgement.

You’ll also bring:

  • An understanding of Home Care Packages and/or the Support at Home Program (desirable)
  • Confidence managing a caseload and taking ownership of client outcomes
  • Strong communication and relationship‑building skills
  • The ability to balance client advocacy, funding requirements and compliance
  • Comfort using digital systems, CRM platforms and care management tools
  • Resilience, adaptability and openness to new ways of working
  • A clinical, allied health or community services qualification (desirable)

About The Good Stuff - The Homewell Way

  • A genuinely supportive culture - Be part of a team where people look out for one another, leaders are accessible, and your contribution is valued. You’ll feel trusted, supported and backed to do your best work.
  • Purpose‑driven work with real impact - Every day, you’ll help older Australians live well at home — shaping care that is personal, meaningful and aligned to what truly matters to them.
  • Learning, growth and professional confidence - Build your capability in an evolving sector through training, mentoring, exposure to reform and access to subject matter expertise. Structured onboarding
  • Reform‑ready ways of working - Clear processes, modern systems and smart digital tools designed to reduce admin and support high‑quality care.
  • Balance and sustainability - Flexible work arrangements and thoughtful workload design that support long‑term wellbeing — because great care starts with supported people.
