The Opportunity:

Reporting to the State Manager the Senior Transitional Support Worker. In this role you will be responsible for assisting with responding to client referrals and assisting with converting the referrals to Claro clients. The Senior Transitional Support Worker will leverage their extensive disability sector knowledge and experience to offer short term support to new Claro clients in need of urgent support.

The Senior Transitional Support Worker will work with a range of clients with various support requirements across your allocated region. You will be given reasonable notice regarding which client you will be support prior to your shift.

This role can based in either Peakhurst Heights, Marrickville or Merrylands areas.

You will be responsible for, but not limited to:

  • Working with new clients to Claro and supporting with their intake an onboarding
  • Occasionally supporting staff to ensure clients goals are being met.
  • As required supporting with
  • Completing client intake and onboarding including completing risk assessments upon meeting new clients.
  • Where required assisting with various complex care tasks
  • As required supporting with activities of daily living in the home
  • Ability to confidentially undertake a handover of supports from hospital settings and apply in the home setting
  • Undertaking client specific training and support other workers to familiarise themselves to new clients

To be successful in this role you will have

  • Minimum Cert III in Individual Support or relevant qualification
  • Extensive demonstrated experience working within the disability field
  • Some experience supervising, supporting, and training staff
  • Ability to travel to various clients with reasonable notice 
  • Can work effectively as part of a multidisciplinary care team  
  • Demonstrated ability to build strong relationships with customers, case managers and other key stakeholders 
  • High level of written and verbal communication skills
  • Ability to work with limited supervision and direction

  • NDIS Worker Screening Check
  • National Police Certificate
  • First Aid and CPR
  • Current Drivers licence
