The Opportunity
Berry Street’s Transitional Youth Support Services (TYSS) supports young people aged 16–25 who are experiencing homeless or at risk of homelessness through assertive outreach, housing support and early intervention.
Within TYSS, the Homelessness Intervention and Prevention (HIP) stream provides early intervention for young people aged 16–18 at risk of homelessness due to family conflict or breakdown.
As a Case Manager, you will deliver housing-focused case management, outreach and early intervention to support young people and families to achieve safe, stable housing, build independent living skills and strengthen connections to family, community and support networks.

- Base Salary $100k pa + Super & Salary Packaging
Your Role
You will:
- Provide housing-focused case management and assertive outreach to young people at risk of or experiencing homelessness.
- Support young people to access, establish and maintain safe and stable housing.
- Assist young people to build independent living skills and develop case and housing exit plans.
- Deliver early intervention support to young people and families experiencing conflict or breakdown.
- Participate in the TYSS outpost, providing crisis response, assessments, referrals and advocacy.
- Undertake risk assessment, safety planning and maintain accurate case records.
- Work collaboratively within a multidisciplinary team.
About You
You are passionate about working alongside young people to support safety, stability, independence and hope. You bring strong case management skills, an understanding of youth homelessness and family systems, and the ability to engage young people in complex and often stressful circumstances.
You will bring:
- Demonstrated experience in case management within the homelessness service system and/or related human services settings.
- A strong understanding of issues impacting young people, including youth homelessness, family conflict and breakdown, trauma and family violence (including application of the MARAM framework, risk assessment and safety planning).
- Well-developed engagement, assessment, planning and advocacy skills.
- The ability to manage competing priorities and stressful situations while maintaining reflective and self-care practices.
- Strong written and verbal communication skills and the ability to maintain accurate case documentation.
- The ability to work collaboratively within a team-based case management approach.
- A Bachelor-level qualification in Social Work, Youth Work or a related human services field.
