The Opportunity
Berry Street is seeking a Case Manager – Residential Care to provide youth-focused and family-sensitive case management to children and young people living in Residential Care.
Working alongside residential carers, Child Protection and other services, you will support young people with complex needs to engage in education, build life skills and work towards their goals. The role involves coordinating care plans, facilitating care team meetings and ensuring young people receive consistent, therapeutic support.

About the Role
We are seeking a Case Manager – Residential Care to provide youth-focused and family-sensitive case management to children and young people living in Berry Street Residential Care homes.
Working closely with residential care teams, Child Protection and external services, you will support young people with complex needs arising from trauma to engage in education, maintain connections with family and community, and work towards their personal goals.
Key responsibilities include coordinating care planning, facilitating care team meetings, maintaining accurate documentation and advocating for the needs of young people to ensure coordinated and effective service delivery.
About You
You are passionate about supporting young people experiencing homelessness to achieve safety, stability and independence.
You bring:
- Experience in case management within homelessness or related human services
- Understanding of youth homelessness, family conflict, trauma and family violence, including application of the MARAM framework
- Strong engagement, assessment and advocacy skills
- Ability to build relationships and coordinate services to support young people with complex needs
- Strong communication and case documentation skills
- Ability to manage competing priorities in fast-paced environments
You will also hold a Bachelor qualification in Social Work, Youth Work or a related human services field.
