About the Home-Based Care program

The Home-Based Care program provides Case Management for Complex, Intensive, General and children supported by a Targeted Care Package (TCP). Most children and young people are placed in out of home care following child protection intervention. A small number are in care on a voluntary basis. Voluntary and statutory placements can be of a short or long term nature.

Your Opportunity

The Case Manager role will be a generalist position within the Home Based Care, Kinship and ICMS program.

They will hold a caseload, providing case management intervention to provide preventative and secondary level support.

About You

The successful applicant will have experience in outreach support and/or case management with young people. You will be required to have the capacity to understand, work and support young people, their family, carers and networks within a community environment and the skills to engage, negotiate and advocate.

Qualification requirements are to have completed a Bachelor qualification in Community Services, Education, Social Work or similar field. You will be a quick learner with high energy and commitment to your clients, yet able to work within the parameters of the role. You will need to be able to work flexible hours to provide a responsive service to your clients.

Experience undertaking contracted case management within DFFH, Child Protection or another similar agency will be looked on favourably.
