The Opportunity

This role involves the planning and managing the Cardiac Services and HITH program under the Better @ Home Project at Benalla Health. The successful applicant will provide leadership in the ongoing development of clinical practices in relation to patients with complex health needs associated with heart failure and HITH Coordination.

 

The successful applicant with provide clinical advice to patients, carers, and other health care professionals whilst monitoring the clinical care of patients transitioning from the acute settings to home by providing clinical support to improve patient care and outcomes. The successful applicant will primarily be responsible for overseeing a co-ordinated approach to nursing care for people with a variety of health conditions including heart failure in order to reduce the demand on hospital services, promote multidisciplinary care and support patients to remain healthier at home for longer.

 

The Role is Part time (0.8 EFT) on a 12 month fixed term contract. EFT will be between the HITH program and Heart Failure program.
