About the Role:

The Primary Health Team Leader provides operational, clinical and professional leadership to the Primary Health team and is responsible for the coordination and delivery of multidisciplinary community and primary health services. The role supports the health and wellbeing of the local community through the provision of accessible, integrated and person-centred care.

About You:

  • Relevant tertiary qualification in Nursing, Allied Health, Community Health or a related discipline.
  • Current registration with AHPRA or the relevant professional body, where applicable.
  • Minimum of three years’ postgraduate experience in community health, primary health, allied health, nursing, or a related healthcare setting.
  • Demonstrated leadership or senior clinical experience within community health, primary health, rural health, or multidisciplinary service environments.
  • Strong understanding of multidisciplinary models of care and contemporary primary health principles.
  • Demonstrated experience in staff supervision, performance development, workforce support and team leadership.
  • Knowledge and understanding of quality improvement, clinical governance, risk management and accreditation requirements.
  • Well-developed communication, interpersonal and stakeholder engagement skills.
  • Ability to work collaboratively and effectively within a small rural health service environment.
  • Demonstrated organisational, problem-solving and time management skills.
  • Current Victorian Driver Licence
