About the Role
We are looking for a suitably qualified Complex Care Coordinator to assist the organisation to meet its strategic and operational objectives in relation to Chronic Disease Management. The Complex Care Coordinator role will contribute to the health enablement of community members of all ages.

About You:
Essential:
- Relevant tertiary qualifications in a health or related field
- AHPRA registration or equivalent
- Demonstrated understanding of the Social Model of Health, health promotion and Primary Care Principles
- Experience in navigating health pathways for clients with complex needs
- Experience in chronic disease management practice
- Excellent organisational skills, ability to prioritise and manage competing demands
- Excellent written and verbal communication skills
- Ability to work in a multidisciplinary team environment and unsupervised.
- Computer literacy skills
- Current Victorian drivers licence
- Prior experience in a project management role
- Understanding of evidence based CDM models
Desirable:
- Understanding of national and local health system pathways
- Experience in a rural or regional healthcare settings
