About the Role

As a Support Worker, you will provide individualized, person-centred support to adults, young people, and children with disability and complex support needs.

You will work alongside participants, families, carers, and community networks to support individuals to achieve their goals while promoting dignity, independence, choice, and inclusion.

We're looking for compassionate, reliable, and values-driven Support Workers to join our team in Cragmuir, VIC


Key Responsibilities

  • Support participants with daily living activities and personal care
  • Assist with household and domestic tasks
  • Encourage independence and skill development
  • Support community participation and social inclusion
  • Implement individual support plans and goals
  • Follow Positive Behaviour Support Plans (BSPs)
  • Administer medication in accordance with policy and individual health plans
  • Accurately complete progress notes and documentation
  • Build positive relationships with participants, families, carers, and stakeholders
  • Ensure participant safety, wellbeing, and safeguarding requirements are met
  • Promote participant rights, dignity, and self-determination

  • You are willing to obtain a First Aid/CPR Certificate, NDIS Worker Screening Check, and Working with Children Check.
