Who are we looking for?
We’re looking for a compassionate, culturally inclusive Service Partner to provide support that helps older South Australians to continue living safely and independently in their homes. You’ll use your skills to guide customers and their families through care plans, answer questions with patience and clarity, and ensure their needs are met, inclusive of any cultural elements. With a balance of empathy and accuracy, you’ll deliver customer-focused assessment, planning, and review to support our elders in achieving their goals.
What can you expect to be doing?
As a Service Partner, you will:
- Deliver quality, culturally safe care management, including care plan implementation and ongoing oversight
- Provide customer service and advocacy, supporting each customer with care monitoring, budget management, and discharge from services in line with organisational procedures and performance metrics.
- Apply a Social and Emotional Wellbeing approach: Support Elders in ways that strengthen connection to culture, community, family, and Country, while promoting independence and daily functioning through wellness and reablement practices.
- Respond promptly to customer enquiries, providing accurate information and guidance to support safe, independent living at home.
- Build strong rapport with customers, carers/families, and advocates to ensure an exceptional service experience.
What do you need to bring?
- Must identify as First Nations and have a strong understanding of the unique challenges that First Nations Elders, people and communities face in Aged Care.
- Must provide a First Nations community reference regarding your skills, knowledge and experience in First Nations service delivery.
- Minimum Certificate III in Individual Support or equivalent relevant qualification.
- Strong understanding of the health, wellbeing, and lived experiences of First Nations people, including culturally safe and strengths-based practice.
- Demonstrated experience building trusted relationships and co-designing supports with First Nations Elders, older people, and their families.
- Ability to deliver and coordinate care in line with individual care plans, policies, and procedures.
- Strong communication and interpersonal skills, with the ability to engage respectfully both face-to-face and over the phone.
- Ability to work with initiative, meet timeframes and KPIs, and collaborate effectively with customers, families, and colleagues.
- Commitment to professional, responsive customer service and advocacy for vulnerable people.
- Working knowledge of aged care legislation, standards, and programs.
