Who are we looking for?
We are seeking capable and values-driven Service Partners to join our Community Aged Care team, supporting older people to live safely and independently at home. Coordinate quality, person-centred care services that promote independence and support customers to achieve their goals through assessment, planning and review.
What can you expect to be doing?
As a Service Partner, you will work alongside customers to implement and monitor support plans, navigate aged care services, and ensure care aligns with individual goals, preferences and funding. You will act as a trusted point of contact - advocating, coordinating and resolving issues to achieve positive outcomes.
- This role requires someone who understands community aged care, balances compassion with accountability, and works confidently within legislation, KPIs and quality standards.
- Coordinate and monitor customer support plans in line with goals, funding and legislative requirements.
- Support customers to navigate the aged care system, promoting wellness and reablement.
- Work collaboratively with customers, families and service teams to deliver quality outcomes.
- Provide advocacy for vulnerable customers where required.
- Deliver responsive customer service while meeting required timeframes and KPIs.
What do you need to bring?
To thrive in this role, you will have:
- Minimum Certificate III in Individual Support (or equivalent).
- Knowledge of wellness and reablement principles in community aged care.
- Understanding of Aged Care legislation, standards and funded programs.
- Experience coordinating services in line with care plans and guiding documentation.
- Strong interpersonal skills and the ability to engage sensitively with diverse customers.
We would also love you to have:
- Experience supporting frail older people with complex needs.
- Confidence working within policy and procedural frameworks.
- Experience working toward KPIs and service timeframes.
- A strong advocacy mindset and commitment to person-centred care
