What you’ll do

As an Aboriginal Care Partner within our Community Aged Care team, you’ll support Aboriginal Elders to live safely and independently while staying connected to culture, community, Country and kinship.

Working primarily across Adelaide’s northern areas, you’ll build trusted relationships with Elders and their families and coordinate culturally safe care that reflects each person’s needs, choices and goals.

You’ll be trusted to make a valuable impact by:

• Developing and reviewing culturally safe, Elder-focused care plans.
• Coordinating services across Support at Home, CHSP and potentially TCP.
• Managing care packages, budgets and service delivery.
• Supporting Elders to understand their options and make informed choices.
• Responding to changes in an Elder’s health, wellbeing or circumstances.
• Building strong relationships with families, community organisations, ACCOs, ACCHOs and other service providers.

What you’ll bring

To thrive in this role, you will have:

• Aboriginal identity and a strong connection to community.
• A minimum Certificate III in Individual Support or a relevant equivalent qualification.
• Experience supporting Aboriginal people and building trusted relationships with Elders.
• An understanding of the experiences affecting Aboriginal Elders, including the ongoing impacts of the Stolen Generations.
• Experience in care coordination, case management or community-based support.
• Strong communication, organisation and relationship-building skills.

We would also love you to have:

• Experience within Community Aged Care or knowledge of Support at Home, CHSP or TCP.
• Experience managing care packages, individual budgets or support plans.
• Knowledge of wellness, reablement and trauma-aware care.
