What you’ll do
As a Practice Leader within our Supported Community Living (SCL) team, you’ll play a key role in strengthening frontline practice and supporting Disability Support Workers to deliver consistent, person-centred support across customer homes.
This role is designed to embed best practice where it matters most - alongside customers and staff in the home environment. You’ll provide hands-on coaching, guidance and supervision while modelling approaches such as Active Support and Positive Behaviour Support to improve everyday outcomes and quality of life for customers.
Working across a cluster of homes, you’ll help build team capability, strengthen communication between shifts and stakeholders, and ensure supports remain safe, meaningful and aligned with NDIS requirements.
You’ll be trusted to make a valuable impact by:
- Providing day-to-day practice leadership, coaching and supervision to Disability Support Workers across SCL homes
- Supporting the implementation of Person-Centred Planning, Active Support and Positive Behaviour Support approaches
- Strengthening communication across teams, shifts, families and key stakeholders to support consistent service delivery
- Monitoring documentation, reporting and practice quality to ensure alignment with NDIS standards and continuous improvement
- Remaining connected to frontline work through regular presence across homes (including occasional active shifts)
What you’ll bring
- A qualification in Disability, Mental Health, Allied Health or significant relevant disability sector experience
- Demonstrated experience supporting person-centred, strengths-based practice in complex support environments
- Practical experience implementing Active Support and Positive Behaviour Support approaches
- Confidence coaching and guiding teams in a supportive, practice-focused way
- Sound understanding of NDIS Quality & Safeguards requirements, including restrictive practice protocols
