What you’ll do

As a Support Facilitator, you will work alongside young people with lived care experience as they transition to independent living. Through strengths-based, trauma-informed case management, you will support young people in their own homes to build confidence, develop practical life skills, and achieve their personal goals.

You will play a key role in helping young people navigate housing, finances, relationships, education, and community connections, while ensuring their voices remain central to every decision. Working collaboratively with families, services, and supports, you will help create pathways toward long-term stability and independence.

This role offers the opportunity to make a meaningful difference by empowering young people to shape their own futures, supported by a values-driven organisation committed to inclusion, reconciliation, and community wellbeing.

This position is a .8FTE, 12 month fixed-term contract.

You’ll be trusted to make a valuable impact by:

● Delivering trauma-informed, client-led case management and support coordination in young people’s homes and communities.

●Supporting young people to set goals, build living skills, and access housing, financial, and social supports.

●Coordinating referrals and advocating for young people across relevant services.

What you’ll bring

To thrive in this role, you will have:

●Relevant qualifications and/or experience in community services, youth work, social work, or a related field.

●Demonstrated experience in case management and supporting young people with complex needs.

● Strong understanding of trauma-informed, strengths-based, and client-centred practice.

We would also love you to have:

● Experience supporting young people transitioning from out-of-home care.
