What you’ll do
This unique Registered Nurse role provides clinical and care management coverage across Community Aged Care when additional support is required. Working across both Clinical Care Partner and Care Partner functions, you’ll ensure continuity of care and support customers to remain living safely and independently at home.
Based primarily from the Western Hub, this role combines office-based coordination with community visits across metropolitan Adelaide. You will conduct clinical assessments, review care plans, support service coordination and respond to changes in customer needs, ensuring care remains safe, responsive and aligned with a wellness and reablement approach.
This role suits an experienced RN who enjoys variety, autonomy and working across both clinical and care management functions.
You’ll be trusted to make a valuable impact by:
- Undertaking clinical assessments, care planning and reviews to support customers with complex or changing needs.
- Providing care management support including service coordination, monitoring care delivery and managing individual budgets.
- Responding to care escalations and changes in health status to ensure appropriate clinical interventions.
- Supporting restorative, reablement and end-of-life care approaches that maximise independence and wellbeing.
- Working collaboratively with families, clinicians, care partners and external providers to achieve positive outcomes.
What you’ll bring
To thrive in this role, you will have:
- Bachelor of Nursing and current AHPRA registration as a Registered Nurse.
- Demonstrated nursing experience within community health, hospital, rehabilitation or aged care environments.
- Experience conducting clinical assessments and managing complex care needs.
- Strong understanding of wellness, restorative and reablement models of care.
- A current driver’s licence and willingness to travel across metropolitan Adelaide.
We would also love you to have:
- Experience within Home Care Packages or community aged care programs.
- Knowledge of end-of-life care and supporting customers with complex clinical needs at home.
- Experience working collaboratively within multidisciplinary teams across health and community services.
