What you’ll do
As a Service Partner in Community Aged Care, you’ll support older South Australians to live safely, independently and confidently in their own homes.
Working in partnership with customers, families, carers and service providers, you’ll coordinate and monitor personalised support plans that reflect each customer’s goals, preferences and available funding. You’ll be a trusted point of contact, helping customers navigate the aged care system, make informed choices and access the services, equipment and supports they need.
This is a rewarding opportunity to build meaningful relationships with customers while balancing compassionate, person-centred support with quality, funding and service requirements.
You’ll be trusted to make a valuable impact by:
- Implementing and monitoring support plans for customers with low-level or self-managed care needs.
- Coordinating services, equipment, assistive technology and home modifications in line with customer goals, budgets and funding requirements.
- Monitoring changes in customer needs and responding through appropriate care escalation processes.
- Supporting customers to navigate the aged care system and make informed decisions about their care and service options.
- Working collaboratively with families, carers, internal teams and external providers to achieve positive customer outcomes.
What you’ll bring
To thrive in this role, you will have:
- A minimum Certificate III in Individual Support (or equivalent).
- Experience supporting older people within community aged care or a similar care environment.
- Knowledge of wellness, reablement and person-centred care principles.
- Understanding of Aged Care legislation, standards and funded programs.
- Strong interpersonal skills and the ability to engage sensitively with diverse customers.
- Experience coordinating services in line with care plans and guiding documentation.
We would also love you to have:
- Experience supporting older people with complex or changing care needs.
- Knowledge of the Support at Home program and the broader aged care system.
- Experience managing individual customer budgets or coordinating funded services.
- Experience working toward KPIs and service timeframes.
- A strong advocacy mindset and commitment to person-centred care.
