What you’ll do
As a Clinical Care Partner, you will support customers with complex and changing clinical needs across Community Aged Care programs. This role focuses primarily on the Transitional Care Program (TCP), helping customers transition safely from hospital to home through short-term, goal-driven restorative care.
Working closely with multidisciplinary teams including nurses, allied health clinicians and service staff, you will coordinate clinical care, provide case management support and ensure the right interventions are delivered at the right time. Alongside restorative care, you will also support customers with complex clinical needs and end-of-life care, ensuring services are delivered safely, efficiently and in line with customer goals.
You’ll be trusted to make a valuable impact by:
- Coordinating care for customers with complex clinical needs across TCP, restorative and end-of-life care programs.
- Supporting hospital-to-home rehabilitation through goal-focused restorative care planning.
- Providing clinical oversight and guidance to multidisciplinary teams including allied health and nursing staff.
- Conducting clinical assessments and escalating risks or changing care needs appropriately.
- Collaborating with internal teams and external services to ensure safe, timely and person-centred care.
What you’ll bring
To thrive in this role, you will have:
- Tertiary qualification in Nursing or Allied Health with current AHPRA (or relevant professional) registration.
- Demonstrated experience in clinical aged care, rehabilitation, restorative care or complex case management.
- Strong clinical judgement and the ability to identify and escalate risks or changing care needs.
- Excellent communication and collaboration skills when working with multidisciplinary teams.
- Ability to work autonomously in a fast-paced environment with changing priorities.
We would also love you to have:
- Experience working within Transitional Care Programs (TCP) or hospital-to-home rehabilitation services.
- Knowledge of aged care legislation, standards and community aged care programs.
- Experience supporting customers with complex conditions including wound care, medication coordination or palliative care.
