Role Overview:
The McGrath Cancer Care Nurse (Breast) will provide leadership in Alpine Health and the community through the delivery of a service to support breast cancer patients across the Alpine Shire which is focused on advocating and coordinating care for individuals diagnosed with breast cancer. Additionally, the position includes community based health promotion, promotion of the McGrath Foundation, group based intervention strategies and innovative strategies to meet individual needs.
Key Responsibilities:
The McGrath Cancer Care Nurse maintains the following primary responsibilities:
- Deliver an Alpine Shire wide service to provide specialised breast care, information, education, psychological support and clinical care to clients relating to breast cancer in accordance with evidence based clinical practice guidelines;
- Assess the individual needs of the patient, their families and carers and co- ordinate relevant support services in hospital and/or the community including palliative care services.
- Work in collaboration with the cancer nurses within the Health Service and greater cancer care network including telephone/telehealth cancer support services.
- Liaison with the oncology, radiation oncology, palliative care and allied health departments ensuring the continuity of care and services to patients with cancer
- Actively facilitate timely communication within the Multidisciplinary team (including Primary Health care providers) relating to treatment plans, clinical and supportive care needs of the patient and their families.
- Provide specialised and relevant education and information on their diagnosis and relevant supportive care resources.
- Provide clinical and supportive care assessments using validated screening tools and techniques within the clinical /optimal care treatment pathways.
- Actively participate in the multidisciplinary team and will be involved in treatment planning.
- Will be available by phone or pager within business hours to answer queries and provide advice regarding the care of patients who have cancer.
- Attendance and participation in McGrath Foundation education and information sharing events.
Essential:
Degree or higher tertiary qualification in nursing with particular relevance to Breast Cancer or oncology including;
- Current Australian Health Practitioner Regulation Agency (AHPRA) registration
- Registered nurse ideally with at least 5 years post registration experience in either surgical or medical oncology, ideally with at least three years’ experience as a breast care nurse, or related discipline;
- Relevant post-graduate qualifications in breast care and cancer nursing (minimum first unit of post graduate certificate, with willingness to complete the post graduate certificate in breast care;
- Nursing or cancer nursing.
Desirable:
- A proficient knowledge in the management and support of patients with breast cancer, their families and carers;
- Previous experience supporting people with breast cancer;
- Previous experience delivering group programs such as Cancer Wellness and Exercise Program;
- Demonstrated ability to work within a multidisciplinary team;
- Demonstrated ability to work as an independent practitioner within a defined support and indirect supervision framework.
