Role Overview:
The Alpine Health District Nursing Service (DNS) is to provide appropriate, cost efficient and contemporary best practice in nursing services within the philosophy and objectives and the policies and procedures of Alpine Health. The District Nursing team provides district nursing care to clients under the Commonwealth Home Support Program (CHSP), Victorian Home and Community Care-Program for Younger People (HACC-PYP), and is a service provider to Department of Veteran Affairs (DVA). District nursing services may also be contracted to provide service to HITH (Hospital in the Home), TCP (Transitional Care Program), Home Care Packaged Clients, Short Term Supports. The District Nursing Service provides quality nursing care to clients in their homes. In this role the District Nurse will be responsible for providing holistic nursing care within the home setting and will require demonstrated skills in clinical assessment, promoting health independence and care coordination along with knowledge related to primary health care
What we are looking for:
- Minimum of three years acute clinical and community nursing experience as a Registered Nurse.
- Passion for Community Nursing.
- Experience in Wound Management, CVAD's, Palliative care, Negative Pressure Wound Therapy, Stoma and Catheter care.
- Ability to work independently and without direct supervision within an MDT environment.
- Ability to problem solve and make decisions in a variety of complex situations.
- Commitment to ongoing professional development.
- Prepared to work weekends as required.
