Role Overview

The District Nurse Coordinator is responsible for the coordination, leadership, and day-to-day management of district nursing services within the community. This fixed-term role ensures continuity of high-quality, patient-centred care during a period of leave, supporting staff, managing service delivery, and maintaining clinical standards.

Key Responsibilities

Clinical Leadership & Coordination

  • Provide leadership and oversight to the district nursing team
  • Coordinate daily service delivery, including patient allocation and workload management
  • Ensure care is delivered in accordance with clinical guidelines, policies, and best practice standards
  • Support complex clinical decision-making and escalation as required

Staff Management & Support

  • Supervise, mentor, and support nursing and support staff
  • Assist with rostering, leave management, and workforce planning
  • Facilitate team communication and contribute to a positive workplace culture
  • Identify training and development needs and support ongoing education

Patient Care & Quality

  • Ensure delivery of safe, effective, and person-centred care in community settings
  • Monitor care plans and outcomes, ensuring appropriate documentation and follow-up
  • Manage patient flow, referrals, and service access
  • Address feedback, complaints, and clinical incidents in a timely manner

Operational Management

  • Oversee daily operations to ensure efficient service delivery
  • Liaise with internal departments and external stakeholders (e.g., GPs, allied health, hospitals)
  • Contribute to reporting, audits, and quality improvement initiatives
  • Maintain compliance with relevant healthcare standards and regulations 

Essential

  • Registered Nurse Division 1, APHRA registration

Desirable

  • Relevant Postgraduate qualification
  • Relevant experience in Community Nursing
