  • Permanent position - Grade 1 or Grade 2
  • Full time (80 hours per fortnight + flexible monthly ADOs)
  • Social Worker Grade 1 Year 1-5 (SC12-SC16, $70,590.00 - $85,872.80) or Social Worker Grade 2 Year 1-4 (SC21-SC24, $90,235.60 - $103,724.40)
  • Located at The Alfred - this role may require you to work at all Bayside Health campuses

The Department

The Social Work Department operates in line with Alfred Health’s values and strategic goals, with a strong focus on enhancing quality of life and supporting patient participation in care.

Social Workers play a key role in identifying and addressing the psychosocial impacts of illness and hospitalisation for patients, families and carers. This includes providing counselling, emotional support, advocacy, service coordination and discharge planning.

The department is committed to ongoing professional development, student education, research and quality improvement. All Social Work staff are required to meet professional practice standards and maintain eligibility for membership with the Australian Association of Social Workers (AASW).

The Role

This role provides social work services across a broad clinical caseload within the health service. The Social Worker will support patients and their families/significant others by delivering high-quality, patient-centred care in a multidisciplinary environment.

Depending on experience and grade, the role may include:

  • Managing a clinical caseload of varying complexity
  • Providing psychosocial assessment and intervention
  • Supporting discharge planning and care coordination
  • Contributing to patient flow and service delivery outcomes
  • Participating in supervision, teaching and learning activities
  • Supporting quality improvement and evidence-based practice initiatives

The successful candidate will demonstrate strong communication skills, the ability to work collaboratively, and flexibility in responding to changing service demands.

Experience and Qualifications Required

Grade 1:

  • Demonstrated ability to provide social work services within a hospital setting
  • Developing skills in assessment, intervention and discharge planning
  • Ability to work effectively within a multidisciplinary team
  • Strong communication and interpersonal skills

This role will require you to provide leave coverage across all three Alfred Health campuses

Grade 2:

Demonstrated experience in delivering social work services in a hospital setting, with a particular focus on Cancer care

Ability to manage a clinical caseload, including patients with more complex needs

Well-developed assessment, intervention and care coordination skills

Ability to exercise professional judgement and work with increasing autonomy

Experience contributing to supervision, teaching or quality activities (desirable)

  • Demonstrated hospital based experience is an essential requirement of this role
  • Bachelor of Social Work (or equivalent)
  • Eligibility for membership with the Australian Association of Social Workers (AASW)
