About the role
Alfred Community Mental Health is committed to improving the physical health of clients with chronic physical health conditions by better the aim of improved coordination of physical health care of clients with co-morbid chronic conditions and in promoting service wide interventions beyond the assessment and screening phase.
This has led to the development of this specialist Nurse Practitioner (candidate) role, aimed at improving the physical health outcomes of clients by promoting health screening, assessment, treatment, care coordination, capacity building and linkage to primary and acute health care.
The Mental Health/Primary Care Nurse Practitioner will support clinical education, mentorship and offer innovative programs to enhance engagement of consumers in therapy and treatment and provide extensions to practice through prescription of medications, ordering of diagnostic investigations, direct referral to other health care professionals, and/or provision of absence from work certificates, within the limitations of their registration as a Registered Nurse.
About you
- Either: Current endorsement by the Australian Health Practitioner Regulation Agency (AHPRA) as a Nurse Practitioner, having completed a Masters of Nurse Practitioner (or a Masters granted equivalence of qualification by AHPRA plus an approved Therapeutic Medication Management module)
- Or: Currently completing a relevant Masters of Nurse Practitioner as a Candidate in the final year within the specialities of mental health and primary care.
- A minimum of 5 years’ experience in a senior nursing role
- Relevant experience in clinical assessment and treatment in the areas of mental health and chronic disease management
- Mandatory: – Current Victorian Driver License and Working with Children’s Check.
- Australian working rights and current Influenza vaccination
