On 1 January 2026, Alfred Health will come together with Kooweerup Regional Health Service, Peninsula Health, Bass Coast Health and Gippsland Southern Health Service to create Bayside Health, a newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.

The Department

HARP is a service within the range of Health Independence Programs (HIP), and reports to the Acute and Integrated Care clinical program, within the HAC program. HARP aims to prevent avoidable hospital presentations and admissions. The Alfred HARP services achieves this through multidisciplinary and interdisciplinary models of care. Providing services to people with chronic needs and or complex psychosocial backgrounds. The service is designed with a central ‘hub’ of discipline specific clinicians and ‘spokes’ of geographically based teams.  

The Role

Hospital Admission Risk Program (HARP) is seeking an experienced Occupational Therapist to join our multidisciplinary team providing complex care in the community to a diverse range of clients. The role of HARP is to promote health independence and optimise health outcomes for patients at risk of hospital admission. We educate, safety net, and connect patients to community resources and primary care and partner with a range of agencies internal and external to the organisation. We operate across three sites within The Alfred Health Group of hospitals (Sandringham, Caulfield and Prahran). The successful applicant will have a passion for complex patient care and significant relevant experience within the acute and community sector as well as excellent collaboration and communication skills. 

Qualifications and Experience Required

  • B. App Sc (OT), B. Occupational Therapy, MOT prac or recognised equivalent
  • Full Registration held with Occupational Therapy Board of Australia (AHPRA)
  • Member of OT Australia (or eligibility for membership)
  • Current Australian Driver’s License (unrestricted)
  • Current Working with Children’s Check
  • Minimum three years’ clinical experience in Occupational Therapy
  • Experience in home visiting, managing caseloads in the community; experience in general medicine and chronic disease management (including recognising and escalating deterioration)
  • Demonstrated best practice in client assessment and intervention
  • Demonstrated ability to evaluate and modify service delivery in accordance with evidence-based practice
