Alfred Health is seeking experienced Occupational Therapists to join our pilot Rapid Allied Health Response service.  The service will support on our ‘virtual hospital’ and home-based models or care.  It will provide a rapid response option (within 24-48 hours) for those seen by Virtual ED and have immediate Allied Health needs.  The aim of the service will be to optimise safety and function and facilitate ongoing care requirements for the patient/family to remain safe and confident in the community. 

 

The Position

The Rapid Response Allied Health Service is part of a pilot program to support Alfred Health virtual and home-based models of care, outside the hospital walls.  It will support the diversion of physical ED presentations, reduce pressure and demand on emergency services and help prevent avoidable admissions or readmissions to hospital.   The service will receive and action referrals by the Virtual ED team, with view to expand the service scope as required.  

 

The service will provide a timely response to referrals, within 24-48 hours, and target our community dwelling, adult patients within the Alfred Health catchment that have immediate Allied Health needs.  The aim of the service will be to optimise safety and function and facilitate ongoing, longer term care requirements for the patient/family to remain safe and confident in the community.  

 

The service will sit within the HARP team and be administered by a grade 3 Physiotherapy or Occupational therapist Allied Health Practitioner (AHP) working within a transdisciplinary scope of practice.  The Rapid Response Service will be multimodal in its delivery via telephone, telehealth and/or outreach community visits. The AHP will work in collaboration with key aligned streams of care including Virtual ED, ED, HARP and ICT.

 

This dynamic role requires clinicians who have great communication, collaborative and care planning skills, with demonstrated leadership experience and who are interested in working and evaluating a new and evolving program for Alfred Health. These roles will be undertaken on a minimum six-month secondment with operational management provided by the HARP team and clinical supervision by an appropriate senior Allied Health Professional.
