Make a Real Difference in Aged Care in Umina as an Enrolled Nurse with Access Telehealth

Access Aged Care is expanding across the Central Coast, and we're looking for experienced nurses who enjoy thinking critically, embracing change and making a genuine impact.

This isn't a traditional aged care nursing role.

You'll work autonomously across residential aged care homes, coordinating care, supporting GPs and specialists, and identifying clinical concerns before they become bigger problems. Every day is different, and as our organisation continues to grow, you'll have the opportunity to grow with us.

What You'll Do

  • Work on site across residential aged care homes as the Access Aged Care nurse.
  • Coordinate care alongside facility nursing teams.
  • Facilitate GP and specialist rounds, both in person and via telehealth.
  • Complete comprehensive clinical assessments.
  • Identify concerns early and coordinate timely intervention.
  • Manage referrals, follow up actions and clinical documentation.
  • Build trusted relationships with residents, families, GPs and facility staff.
  • Confidently use multiple technology platforms to coordinate care and improve clinical outcomes.

What You'll Bring

  • Diploma with current AHPRA registration and at least two years' post graduate experience.
  • Experience in aged care or a similar clinical setting.
  • Strong clinical judgement and confidence making independent decisions.
  • Excellent communication and relationship building skills.
  • Confidence using technology and the ability to quickly learn new systems.
  • A flexible, growth mindset with the willingness to work across multiple residential aged care homes as operational needs change.
  • Strong organisational skills and the ability to prioritise competing demands.
  • Current driver's licence and reliable vehicle.
