Access Aged Care delivers consistent face to face GP support to aged care residents across Australia.
Our nurses work alongside visiting GPs inside aged care homes, identifying changes in resident health early and helping ensure the right care happens quickly. This model strengthens communication between residents, families, facility teams and doctors, helping lift the standard of care.
We focus on practical clinical support, clear communication and strong relationships that improve clinical outcomes for aged care residents.
We are currently looking for a Registered or Enrolled Nurse to join our team onboarding a new aged care home based in Cooranbong.
This role is ideal for a nurse who enjoys working autonomously, building strong relationships with residents and care teams, and applying clinical judgement in a collaborative environment.
You will work closely with our visiting GPs to support care reviews, follow up on clinical concerns and help ensure residents receive the right care at the right time.
About the role
- Provide on site clinical support during GP care reviews and follow up visits.
- Facilitate telehealth consultations with geriatricians and other specialists.
- Identify changes in resident condition and escalate appropriately.
- Build rapport with residents and families and keep them informed about care plans.
- Complete clinical documentation, follow ups and assist with audits or incident reviews.
- Work collaboratively with facility teams, supported by a Nursing Regional Manager and admin staff.
- Gain exposure to different aged care environments and broaden your clinical experience.
- No manual handling. The role focuses on clinical coordination and resident advocacy.
What you will bring
- Current AHPRA registration as a Enrolled/Registered Nurse.
- Minimum two years post graduate nursing experience.
- Strong clinical judgement and critical thinking.
- Clear, calm communication skills when working with residents, families and care teams.
- Confidence working autonomously while remaining highly collaborative.
- Strong organisational skills and attention to detail.
- Comfortable using technology and clinical systems.
- Reliable vehicle and ability to travel locally across the Central Coast.
